WebThe application process for all ID cards is initiated by Human Resources who will: 1. Complete the application form at Appendix 2 with new applicants 2. Take a photograph of new applicant 3. Produce the photographic ID badge (without access permission) 4. Provide ID badge to applicant at Trust Induction which is to be signed for 5. WebIf you have not successfully completed the online automatic verification, you will be required to supply a photograph, proof of address and proof of identity in support of your Blue Badge application. It is unlikely that applicants under the age of 18 will successfully complete the online automatic verification.
